*Benefit Eligible Employees working 20+ hours per week ages 19-70 can apply with Guarantee Issue up to $150,000 of Life Insurance Benefit and ages 71-80 can apply up to $50,000 Life Insurance Benefit subject to medical underwriting. All applicants are required to complete 4 Underwriting questions at time of application – the answers to these underwriting questions will not be reviewed by Chubb unless group participation requirements are not met. Spouses require Underwriting consideration for all levels of coverage.
